This matched cohort study represents one of the first to quantitatively assess health insurance instability following firearm injury using large-scale commercial insurance claims data across a 15-year span. The findings reveal that nonfatal firearm injury survivors encounter disproportionately higher rates of insurance instability compared to matched populations without such injuries. Importantly, the risk is accentuated with greater injury severity.

Insurance instability disrupts continuity of care, which is especially critical post firearm injury due to complex needs including physical rehabilitation, surgical care, and mental health support for trauma and post-traumatic stress disorder. Gaps in coverage may lead to delayed or foregone care, exacerbating morbidity, increasing hospital readmission rates, and ultimately elevating long-term healthcare costs. Furthermore, increased insurance instability shifts the financial burden towards public insurance programs and uncompensated care sectors when uninsured individuals seek emergency or chronic care services.

From a public health perspective, these findings emphasize the necessity for targeted policy interventions aimed at stabilizing insurance coverage for firearm injury survivors. Provisions such as expanded Medicaid eligibility, insurance marketplace support, and employer-based coverage protections could mitigate disruptions. Clinicians should be aware of the high risk of insurance turnover and integrate social work and case management resources in trauma care planning.

Overall, this study highlights a critical healthcare system gap impacting a vulnerable patient population. Addressing insurance instability may improve clinical outcomes and reduce the societal costs associated with firearm injury.
