1. For AF-related stroke, continued emphasis on screening for atrial fibrillation in at-risk populations, widespread use of anticoagulants, and management of cardiac comorbidities remain essential.

2. For diabetes-related stroke, expanding prevention programs addressing obesity, promoting physical activity, healthy diets, and early identification of glucose abnormalities are key. Additionally, integrated care pathways managing multi-morbidity in diabetic patients can mitigate vascular complications.

3. Health systems particularly in middle to high SDI countries should prioritize resource allocation to manage the dual challenge of AF and rising diabetes-associated stroke.

Ischemic stroke continues to be a global health challenge with substantial morbidity and mortality. This comprehensive analysis underscores the rising dominance of diabetes as a driver of ischemic stroke worldwide, in contrast to the declining trends in AF-related stroke. Addressing this evolving burden requires age- and country-specific interventions that integrate primary prevention, risk factor control, and optimized clinical management, particularly with emphasis on tackling obesity and diabetes.

Continued surveillance using updated global datasets will be vital for monitoring these trends and evaluating the effectiveness of interventions. Research into novel therapies targeting diabetes-induced vascular damage and personalized approaches to stroke prevention could further improve outcomes. Understanding the interplay between AF, diabetes, and other cardiovascular risks at a mechanistic level may unlock new preventive or therapeutic strategies.
