Digitally Enabled QI Intervention Modestly Improves LDL-C Control in ASCVD: SAPPHIRE-LDL Trial
The SAPPHIRE-LDL trial investigated whether a digitally enabled, multifaceted quality improvement (QI) intervention could improve LDL cholesterol (LDL-C) control in patients with established atherosclerotic cardiovascular disease (ASCVD) compared with routine practice.
Cluster randomized trial in 28 Brazilian outpatient clinics enrolled 1465 patients with ASCVD.
Intervention included previsit screening, clinical decision support, audit/feedback, and patient engagement tools.
At 6 months, mean LDL-C was 76.3 mg/dL in the intervention group vs 85.6 mg/dL in the control group (adjusted difference −6.62 mg/dL; p=0.004).
More patients achieved LDL-C <50 mg/dL (23.5% vs 13.4%) and ≥50% reduction (18.6% vs 13.4%).
No significant difference in major cardiovascular events was observed.
