Study Background

Voice characteristics, notably pitch, are critical markers of gender identity and perception. For transgender women, achieving a voice pitch consistent with feminine norms is a significant aspect of social gender affirmation. Traditional voice feminization techniques include behavioral speech therapy and various surgical interventions targeting vocal fold tension and length. However, many existing surgeries have limitations regarding the degree and stability of pitch elevation.

Tensioning glottoplasty introduces a biomechanically distinct technique by combining anterior vocal fold mucosal excision (shortening the vibrating length) with sutures that apply anteriorly directed tension externally via the thyroid cartilage. This method is proposed to augment longitudinal tension of the vocal folds, theoretically enhancing pitch elevation beyond what shortening alone can achieve.

Study Design

This retrospective cohort study analyzed 20 transgender women, ages 22 to 41 years, who underwent tensioning glottoplasty at a specialized laryngology center. Patients received surgery combining endoscopic anterior glottic mucosa resection with external transcartilaginous sutures tied over the thyroid cartilage. The key endpoint was change in fundamental frequency (fo) measured acoustically at baseline and one month postoperatively.

Statistical analysis was performed using paired-samples t-tests after confirming data normality with Shapiro-Wilk testing. The degree of pitch change was quantified with mean differences, 95% confidence intervals, p-values, and effect size (Cohen’s dz).

Key Findings

Postoperatively, the mean fundamental frequency rose substantially from 126.2 ± 22.6 Hz to 241.2 ± 34.1 Hz, reflecting a mean increase of 115.0 Hz (95% CI: 102.19–127.87 Hz). This pitch elevation was highly statistically significant (p < 0.001) with a very large effect size (Cohen’s dz = 4.19), underscoring the robustness of the intervention.

All patients exhibited pitch elevation, indicating consistent procedural efficacy. The mean postoperative fo approximates the typical female speaking fundamental frequency range (around 200-250 Hz), which is a clinically meaningful outcome for voice feminization.

The combined surgical technique that integrates vibratory length reduction with tension augmentation may biomechanically explain the improved voice outcomes compared to techniques focusing on either component alone.

Tensioning glottoplasty represents an innovative advance in gender-affirming voice surgery by addressing both vocal fold length and tension simultaneously, key determinants of pitch. By applying external suture tension over the thyroid cartilage, surgeons can modulate vocal fold biomechanics in a controlled manner, potentially enhancing pitch elevation stability and longevity.

Limitations of the study include its retrospective design, relatively short follow-up period (limited to 1 month), and absence of a comparative control group or long-term voice quality and patient-reported outcome assessment. Future prospective, randomized studies with longer-term follow-up and voice function analysis are necessary to validate durability and voice quality improvements.

Nevertheless, this preliminary evidence strongly supports tensioning glottoplasty as a promising surgical option within the multidisciplinary management of transgender voice feminization where prior methods may have provided suboptimal pitch elevation.

Tensioning glottoplasty offers a biomechanically rational, technically feasible, and clinically effective surgical approach to voice feminization for transgender women. The procedure achieves substantial pitch elevation by combining anterior vocal fold shortening with external tensioning, producing fundamental frequencies in the female normative range.

It expands the armamentarium of voice feminization surgeries by targeting multiple biomechanical parameters of vocal fold vibration. Further research is warranted to assess long-term outcomes, vocal quality changes, and patient satisfaction to optimize preoperative counseling and surgical planning.
