While financial barriers are frequently cited as the primary obstacle, recent research suggests that the drivers of this disparity are far more complex and structural.
In many clinical settings, the transition from curative to comfort-focused care requires a level of health literacy and institutional navigation that is not equally distributed across social classes. Low-income patients often navigate a healthcare landscape where informational, psychosocial, and cultural factors converge to discourage the pursuit of hospice. Understanding these drivers is essential for clinicians and policymakers aiming to close the gap in end-of-life care quality. This article examines a recent study that explores the intersection of cultural capital, stigma, and class-based disadvantage in the context of hospice access for cancer patients.
Study Design: A Qualitative Exploration of Barriers
The study, conducted by Yan and colleagues and published in JAMA Network Open, utilized a descriptive qualitative design to investigate the experiences of low-income patients with advanced cancer. The research was situated at a tertiary hospital in southwestern China between July 2024 and July 2025.
