There are several classification systems for diabetic foot, including the commonly used Wagner, Texas, PEDIS, and the simplified classification system. The simplified classification system is relatively simple and easy to use.

Grade 1, Normal Foot

No neuropathy or vascular disease

Grade 2, High-Risk Foot

Presence of neuropathy and vascular disease, plus risk factors such as edema and foot deformity

Grade 3, Ulcerative Foot

Ulcer formation

Grade 4, Infected Foot

Foot infection

Grade 5, Necrotic Foot

Gangrene

Grade 6, Unsalvageable Foot

Possible amputation
