Clinically manifest chronic obstructive pulmonary disease treated with maintenance inhalers.
Documented heart failure diagnosis or LVEF <40%.
Office blood pressure >=140/90 mmHg on at least 2 occasions, or current antihypertensive therapy.
Documented diagnosis, laboratory criteria, or glucose-lowering medication use.
Any prior ischemic stroke or transient ischemic attack.
Prior myocardial infarction and/or peripheral artery disease, including revascularization.
